New Patients
Before your first visit with Dr. Hariri, please print out and fill in the New Patient Intake and Privacy Policy Forms. You must sign the bottom of each of these.
Depending on your area of pain (knee or shoulder), print out the appropriate questionnaire as well. It will save you time if you bring these, filled out, to your appointment.
It is helpful if you bring the printed films or a CD of any previous knee or shoulder imaging (e.g. X-rays or MRI) along with the radiology report to your appointment. If you have already had a surgery, a copy of your operative report from the surgeon’s office is also very helpful.
Returning Patients
If you are seeing Dr. Hariri for a new problem, please fill out the appropriate questionnaire (knee or shoulder).
